Appointments:
We reserve the right to charge $25 for appointments cancelled without 24 hour advanced notice.

If you are more than 10 minutes late for an appointment there may be a $25 charge, and the appointment may need to be rescheduled.

Payment:
Per our contract with your insurance we are required to collect your Co-pay at the time of service. Your Co-pay will need to be paid in full before you see the doctor. We may have to cancel your appointment if payment is not made. Also, for unpaid balances on your account, we require a minimum of 50% payment on the outstanding balance. (in addition to your co-pay)

FMLA or disability forms

Prescription Medications:

No prescriptions will ever be dispensed by this office until a patient has been seen by this office.
Prescription refills are handled Monday, Tuesday, and Wednesday from 9am to 4pm.
Prescriptions will not be filled Thursday, Friday, Saturday, or Sunday.
No walk in appointments for prescription refills will be accommodated.
Prescription refills will only be approved for 3 months, or 1 month for any controlled substance, after which an appointment is required.

If you need to change the type of medication you are taking an appointment with Dr. Hemler is necessary.
If you loose or misuse a prescription we will not authorize a refill without an appointment.
No refill authorizations will be called or faxed to a pharmacy.
We are not responsible for getting your mail in prescriptions to your mail in pharmacy.
